TUBERCULOSIS IN THE REPUBLIC OF BELARUS

24 March - World Tuberculosis Day. Celebrated by the decision of the World Health Organization (TRAIN) in a day, in which in 1882 r. German microbiologist Robert Koch announced his discovery of the causative agent of tuberculosis.

IN 1993 r. WHO declared tuberculosis a national disaster, and day 24 March - World Tuberculosis Day.

Treatment success rate among new TB cases is high, but 1,5 million people die every year from this treatable disease.

Every year they appear 9 million new cases of tuberculosis, but still 3 million cases or were not diagnosed, did not undergo treatment, or the diagnosis has been made, but not registered by national TB programs. Therefore, significant efforts are required to, to close this gap.

After discussions and consultations with partners, STOP TB partnership announced the theme of World Tuberculosis Day 2015 of the year. This topic will be continued with 2014 of the year – “Identify 3 million". The main slogan for this year will be “Identify, treat and heal everyone".

For World Tuberculosis Day in 2015 year, partners will continue to call for global efforts, to reveal, treat and heal all patients, patients with TB and accelerate progress towards the ultimate goal of eradicating tuberculosis by 2035 year.

30-31 March 2015. in g. Riga will host the First Ministerial Conference of the Eastern Partnership to combat tuberculosis and multidrug-resistant tuberculosis, which is a high-level international forum, at which it is planned to discuss international approaches in the fight against tuberculosis.

The above-mentioned Forum will be held under the auspices of WHO and the Global Fund to Fight AIDS, tuberculosis and malaria (GFSTM). Ministers of European countries will take part in the conference, representatives of the European Union, Eastern Partnership member states, international structures and non-governmental organizations.

Planned, that conference delegates will adopt the Riga Declaration on Tuberculosis.

Main activities of the National Tuberculosis Control Program in 2014 r. were aimed at strengthening the regulatory framework, organizational and methodological, information base of healthcare organizations, providing anti-tuberculosis care to the population, implementation of modern effective and economically feasible technologies to combat tuberculosis into practical activities.

For this purpose in 2014 mastered within the framework of the State program 34 851,1 million. rub., including: at the expense of the republican budget - 21 728,8 million. rub. (Ministry of Health of the Republic of Belarus – 19 025,4 million. rub., Ministry of Internal Affairs of the Republic of Belarus – 2 703,4 million. rub.); at the expense of local budgets – 13 122,3 million. rub.

Moreover, additionally allocated for the implementation of State Program activities 10 888,5 million. rub. from the GFATM grant funds.

Developed and approved 6 regulatory documents, aimed at optimizing the provision of TB care and preventing the spread of multidrug-resistant tuberculosis (MLU-TB).

In 2014. year carried out 10 republican seminars (meetings) on various aspects of the fight against tuberculosis: Conducted 34 regional training seminars on various aspects of the fight against tuberculosis.

Prepared and carried out 13-14.11.2014 r. international scientific and practical conference “Introduction of new approaches in the fight against MDR/XDR-TB in Belarus” with the publication of conference materials and its coverage in the media.

The international technical assistance program “Implementation of the STOP-TB strategy in Belarus” is being successfully implemented, in particular, aimed at measures to combat multidrug-resistant tuberculosis (MLU-TB)», financed by GFATM funds.

Events prepared and carried out, dedicated to World Tuberculosis Day (24 Martha). Performances were held on the leading television channels of the republic, and also on Belarusian radio, materials on the fight against tuberculosis were published in printed periodicals, press conference held, took part in the round table meeting, dedicated to the problem of antibiotic resistance. Campaigns for early detection of tuberculosis were carried out in all regions of the republic and. at the Department of Therapy of the Medical Faculty of the Belarusian State University, an associate professor course of tuberculosis was opened.

Constant interaction was ensured between the medical services of the Department of Execution of Punishments of the Ministry of Internal Affairs of the Republic of Belarus and healthcare organizations of the Ministry of Health system, providing anti-tuberculosis medical care. All penitentiary institutions are assigned curators from anti-tuberculosis organizations. Conducted 4 joint training seminars.

The republic has created the necessary prerequisites to increase the efficiency of identifying drug-resistant tuberculosis. PCR diagnostic laboratories have been created in all level III laboratories, who conduct drug sensitivity studies of Mycobacterium tuberculosis (MBT) during 1-2 days, which makes it possible to timely prescribe adequate chemotherapy regimens to patients with MDR-TB. A new algorithm for diagnosing tuberculosis and MDR-TB using rapid methods has been developed. The experience of using rapid methods of molecular genetic diagnosis of MDR-TB shows their high efficiency, which allows for a quick diagnosis, quickly isolate the patient in a specialized department and begin adequate treatment.

All TB organizations have developed and are implementing infection control plans, personal protective equipment is purchased. The electronic registry “Tuberculosis” is functioning, in which, in addition to components on MDR-TB and laboratory diagnostics, in 2014 r. the “Drug Management” component is included.

Republican and regional councils on multidrug-resistant tuberculosis worked effectively. Thanks to the work of the councils, all patients received adequate chemotherapy regimens. On the basis of the state institution "Republican Scientific and Practical Center of Pulmonology and Phthisiology" 5 143 the patient.

The measures taken had a positive impact on the epidemic situation regarding tuberculosis. According to preliminary data, tuberculosis incidence rate taking into account all departments in 2014 r. decreased compared to the same period 2013 r. on 9,9 % (from 38,3 to 34,5 on 100 000 population). Mortality from tuberculosis decreased by 19,0 % (from 5,8 on 100 000 population in 2014 r. to 4,7 on 100 000 population in 2014 g.)

The question on the progress of implementation of the State Program was heard at the board of the Ministry of Health (resolution of the board of the Ministry of Health dated 26.11.2014 r. № 28.3 “On the work of the anti-tuberculosis service of the republic and the implementation of the State program “Tuberculosis” for 2010-2014 years").
